These Terms of Service ("Terms") govern your use of the website located at hpnp.health and any related services provided by Heather Pisano NP in Family Health and Psychiatry, PLLC, doing business as hp | np Health ("Practice," "we," "our," or "I"). By accessing or using this website, you agree to be bound by these Terms. If you do not agree, please discontinue use of the site immediately.

These Terms apply to all visitors, prospective patients, and others who access or use the website. They do not replace or supersede any separate patient agreement, consent form, or treatment contract you may sign as part of becoming a patient of this practice.

1. Nature of This Website

This website is provided for informational purposes only. The content on this site — including text, graphics, and other materials — is not intended to constitute medical advice, diagnosis, or treatment. Nothing on this website creates a provider-patient relationship between you and Heather Pisano, NP-C, or hp | np Health.

A provider-patient relationship is established only upon completion of an intake evaluation, execution of a patient agreement, and mutual agreement to proceed with care. Visiting this website, submitting an inquiry form, or exchanging emails does not constitute the establishment of a clinical relationship.

2. Not an Emergency Service

If you are experiencing a psychiatric or medical emergency, please call 911 or go to your nearest emergency room immediately. This website and its contact methods are not monitored 24 hours a day and are not appropriate for emergency communications.

3. Eligibility and Geographic Limitations

Clinical services provided by hp | np Health are available only to individuals who are physically located in the State of New York at the time of service, consistent with Heather Pisano's licensure as a Nurse Practitioner in New York State. Telehealth services are subject to applicable state telehealth laws and regulations.

By using this website and requesting services, you represent that you are at least 18 years of age, or that you are a parent or legal guardian of a minor for whom you are seeking services. You further represent that you are legally authorized to enter into these Terms.

4. Telehealth Services

hp | np Health may provide clinical services via telehealth (secure video or telephone). By participating in telehealth services, you acknowledge and agree to the following:

  • Telehealth involves the electronic transmission of medical information and carries inherent risks, including the possibility of technical failures or interruptions
  • You are responsible for ensuring you have a private, secure environment for telehealth appointments
  • Telehealth is not appropriate for all clinical situations; in-person evaluation may be required at the provider's discretion
  • You consent to the use of HIPAA-compliant telehealth technology for the delivery of care
  • You may withdraw consent for telehealth at any time by notifying the practice in writing

5. Fees, Payment, and Cancellation Policy

All fees for services are due at the time of service unless otherwise agreed in writing. hp | np Health is an out-of-network provider and does not bill insurance directly. Superbills are available upon request for submission to your insurance carrier.

Accepted Payment Methods

We accept major credit and debit cards, HSA, and FSA cards. Payment is processed securely through our patient management platform.

Cancellation and No-Show Policy

We require at least 48 hours' notice to cancel or reschedule an appointment. Late cancellations (less than 48 hours' notice) and no-shows may be subject to a cancellation fee. Specific fee amounts will be provided in your patient agreement at the time of intake.

Prescription Costs

The cost of medications, laboratory tests, and compounded preparations is separate from and in addition to clinical service fees. These costs are the responsibility of the patient and vary by pharmacy, insurance coverage, and the specific medication prescribed.

6. Intellectual Property

All content on this website — including text, graphics, logos, images, and the overall design — is the property of Heather Pisano NP in Family Health and Psychiatry, PLLC, or its content suppliers, and is protected by applicable copyright and intellectual property laws.
